Experienced Hospice Chaplains provide spiritual care for terminally ill patients at home, in a facility, community settings, or home or community setting, wherever the patient lives. They assist with effective education, spiritual leadership and support to patients and families during patient decline and death. They also assist with bereavement, land funeral and memorial services after the patient passes away.
These Hospice Chaplains provide a unique Spiritual Plan of Care with activities that are designed to significantly benefit each patient and their family. As adjustments must be made during health decline, the plan continues effective spiritual activity to build inner strength, manage pain and increase feelings of comfort and peace.
Assessments, care plans, charting, calls and reports are made as requested by the Hospice Service Provider. Patients are supported in their faith tradition.
Specialized spiritual care interventions and activities can help decrease suffering and increase perspective and peace during final days.
